Role objectives

• a) Provide on-site technical support and supportive supervision to Health Visitors (CHWs) deployed in assigned municipalities and PHC catchment areas.
• b) Monitor the interventions delivered by CHWs against the agreed service package, standard operating procedures (SOPs) and quality benchmarks.
• c) Ensure timely, complete and accurate data collection from CHWs at household and community level, in line with the national CHW data collection tools.
• d) Collect, verify and consolidate reports submitted by CHWs and PHC focal points on a weekly and monthly basis.
• e) Ensure that CHW data is uploaded and reported through the dedicated national reporting application linked to DHIS2, with data quality checks performed prior to submission.
• f) Support PHC facilities to incorporate CHW planning, implementation and reporting as an integral component of their PHC service delivery, including inclusion of CHWs in PHC micro-plans, outreach calendars and monthly review meetings.
• g) Capture success stories, human-interest narratives and lessons learned from the field, and share them with the Programme Management Unit.
• h) Deliver on-the-job mentoring, coaching and refresher orientation to CHWs on service delivery, data recording, referral pathways and community engagement.
• i) Identify bottlenecks and operational challenges at CHW, PHC and municipal levels, and propose corrective actions to the supervisor.
• j) Contribute to cross-regional learning by participating in joint review meetings between West and East Region field monitors and the central Programme Management Unit.

Project reporting

The Field Monitor reports to the Community Health Specialist (West / East & South Region), with technical reporting to our clients Health Officer / CHW Programme Lead. Monthly progress reports and monitoring logs are submitted to the supervisor for review and endorsement.

Team management

No direct line management. The Field Monitor coordinates day-to-day work with Health Visitors (CHWs) and PHC focal points in the assigned catchment areas.
